The effect of pubertal delay by GnRH agonist in GH-deficient children on final height.

چکیده
Several strategies can be applied to optimize GH treatment in GH-deficient (GHD) children during puberty (1). In a recent issue of JCEM, Mericq et al. (2) reported their results of a prospective, randomized trial on the effect of GH alone or in combination with GnRH agonist (GnRHa) in pubertal GHD patients. They concluded that delaying puberty by GnRHa led to a near final height (FH) sd score of 1.3 0.5 compared with 2.7 0.3 in the group treated with GH alone. However, in this study, the patients had been untreated up to the age of 12–18.5 yr, which is unusual for western countries. In addition, in several cases the growth retardation was less severe than would be expected in classical GHD patients without treatment. Although a randomized controlled trial in a large and representative sample is certainly the best design to study the efficacy of a therapeutical regimen, a patient series with matched controls is a good second. We report on a retrospective analysis of the effect of the addition of GnRHa (shortly after the onset of puberty) to GH treatment in GHD children. Matched controls with GH treatment were used for comparison. The children were younger as well as shorter for age compared with the subjects in the randomized trial (2).

Whether all patients with Central precocious puberty should be treated?
Extended Abstract Precocious puberty is the appearance of secondary sexual characteristics before 8 years old in girls and 9 years old in boys. Precocious puberty is divided into 3 groups of central, peripheral and normal variants. Central precocious puberty is accompanied by activation of hypothalamic-pituitary-gonadal axis which causes increase in secretion of GnRH, and in turn increase i...

It has been reported that the final height in short children is strongly related to the height at the onset of pubertal development, and pubertal heiht gain in GH-treated children is not exceed the gain in normal children. Therefore, it is now the consensus that insufficient height at the onset of puberty leads to short final height. We have already demonstrated that the final height in GH-defi...
متن کاملBenefit of postponing normal puberty for improving final height.
Experiments of nature and clinical observations have provided indications that postponing puberty may increase final height in short children. In children with central precocious puberty, a GnRH analog (GnRHa) alone is efficacious in increasing final height, but in other conditions a combination of growth hormone (GH) and GnRHa is needed. In GH-deficient children with early onset of puberty and...
متن کاملBone development during GH and GnRH analog treatment.
Estrogens, GH and IGFs are essential in the development and growth of the skeleton and for the maintenance of bone mass and density. Treatment of precocious puberty with GnRH analogs (GnRHa), by reducing sex steroid levels, leads to a situation of hypoestrogenism that may theoretically have a detrimental effect on bone mass during pubertal development. A reduction in bone mineral density (BMD) ...
متن کاملManagement of precocious puberty
The objectives of treatment for children with central precocious puberty (CPP) are to avoid psychosocial problems caused by early pubertal development and to normalize adult height (AH). A long-acting GnRH analog is the treatment of choice for CPP. GnRH analog administration effectively arrests further development of secondary sex characteristics, slows bone age (BA) maturation, increases puber...
